The forms are optional You may also notify us of your withdrawal from the contract or submit a product claim in another clear way, for example by emailing info@berry-home.cz.

How to Use a Form

1. Download the correct form

Choose the Withdrawal Form or Claim Form depending on the matter you need to resolve.

2. Complete the required information

You can complete the form electronically or print it and fill it in by hand.

3. Submit the form

Email the completed form to info@berry-home.cz or include it in the parcel containing the returned goods or the product covered by your claim.

For faster processing

We recommend always including your order number. When submitting a claim, please also attach photographs of the defect or damaged packaging, where possible.
